New Patients

Thank you for your interest in joining Bayside Medical. We are continuously receiving a large volume of registration requests and as a result are operating at near capacity. To ensure that we can continue to provide safe and timely care for our existing patients, from time to time we need to close our lists to new patients.  If you would like to be added to our waiting list, please complete the registration form below in order for us to best assess your needs.

The registration form will be reviewed. Once registration has been approved, patients will be required to sign a request to transfer medical records from their previous GP. Registration is complete upon receipt of those records and routine appointments can be scheduled thereafter.

If multiple family members are looking to join, please advise for all individuals separately.
